
Hospital Information System Implementation in Francophone Africa
This doctoral thesis provides an in-depth examination of hospital information system implementation across French-speaking African countries, with particular emphasis on practical deployment in Mali. The work addresses the core obstacles that arise when establishing healthcare IT infrastructure in resource-constrained environments—cultural adaptation, political feasibility, organizational readiness, and the technical realities of limited connectivity and infrastructure. The thesis documents Cinz@n, an open-source hospital information system that was developed, tested, and validated directly within Malian healthcare settings. Core technical content covers system architecture design, patient identification protocols aligned with international standards, semantic coding frameworks for medical diagnoses and clinical procedures, workflow-oriented implementation approaches, and proven methodologies for rolling out such systems in challenging contexts. The thesis undertakes a rigorous comparison between established commercial platforms—Cerner and McKesson—and open-source alternatives, making a detailed case for why adaptable, locally-maintainable solutions often outperform proprietary offerings in developing healthcare economies.
